This form, once submitted, will be delivered directly to the executive director. That form will then be shared with the direct report and campus pastor overseeing the area involved. Printed forms can be requested in person at the campus offices or on the website under Inside the Tabernacle. All incidents are expected to be reported within twelve (12) hours and information should remain confidential at all times.
